School Forms

Kindergarten (and new students who are not from Pennsylvania) must submit a current Dental Exam Form and Physical Exam Form with lead test results before the first day of school.

Students entering grade 6 will need to submit a Physical Exam Form completed by a physician before the first day of school.

Students entering grades 3 and 7 will need to submit a Dental Exam Form before the first day of school.
